Ichigo's full hollow form is actually a hollow in the Vasto Lorde Stage, with the power much greater than the Espada. Hence it's known as the Vasto Lorde form. This form of his arose from his suppressed hollow powers, when he had a hole blown in his chest by Ulqiorra's Cero-Oscuras.
